CCME Services, powered by Constellation Quality Health, and the Division of Health Benefits (DHB) have developed a secure electronic submission process as the required format for submitting Prior Authorization requests. The web based tool additionally allows providers to:
Track status of requests
View and print authorization notices
Respond with additional information
View instructional and training materials

The data on this website is confidential, and the system logs all activity. This system is for business use only by authorized users. Provider Registration
All users must register (Create an account) for access to the ChoicePA web portal. Registration is completed electronically for validation and immediate log in ability.
Get Started in 3 Easy Steps:
1.  Each Provider (NPI) identifies one or two ChoicePA Administrators (Admin)
2.  The Admin creates an account and establishes a security PIN
3.  Other users register using the security PIN from the Admin
